Ugh, Dr.'s who think everyone is trying to do shady chit. Tweaked my back 2 weeks ago and missed 3 days of work. Went to the Dr and had a work release. I have Aflac and have used it multiple times without issue. So when filing this claim for missed time at work, it is required to have myself fill out a form, my work fill out a form and the physician to fill out a form. Did it back in 2011 or 2012 when I wrecked my 4 wheeler and wasn't an issue. Went and got everything from patient records, started my claim and downloaded the forms. Took the physicians form to the express care I went to a dropped it off. They called about an hour later and asked a few questions then said OK, I'll get this finished up and have it at the front desk. 15 minutes later they call back. Her manager says they do not fill out these forms, due to Aflac calling it a disability form, only statements of treatment. Tried to explain it was only stating that I missed work and was supplemental insurance coverage for such instances. Didn't matter she has to tear the form up and I can take it up with her manager. So now I have to call my afalac agent tomorrow, figure out what the hell is going on and what to do about it. I have all the physician notes and the work release but the form requires codes of diagnosis and other "physician" things. Last time I use Memorial for anything if they still refuse. My only hope is when I started the claim it stopped after staing missed work and had a link for downloading the forms, it errored out so I went to their download center and printed what I thought was correct.
Shark Week!!! Good grief, how much new stuff can you have to warrant a full week every year on it. Everything they hype, Phelps racing a shark, whatever happemed with Shaq last year(didn't watch) end up fake and nowhere near what is hyped and all the BS hypothetical fake tales about smart sharks etc... It's run it's course, people are over it. Bring back normal programming
Requires CODES of diagnosis. There is the problem. From my recent two year health issues with my kids, coding issues are a major issue in preventing care. Too damn complicated.
Yeah, it's not so much the codes that are the problem it is the disability word on the banner of the form. I think they are thinking I am seeking some form of disability claim with the government or suing. I'm hoping somehow I got the wrong form. I'll let the Aflac agent handle it. I have everything I need for proof except for that portion, otherwise I'm just going to cancel it. If going to the Dr. is required to use it, and a Dr. can refuse to fill out the needed paperwork whenever they want for any reason it kind of makes it not guaranteed. This is the first time I have ever had issue with it.
I rarely use my insurance, unless it's major. The hoops they make you jump through are alot less when you hand them cash haha. Not really option for everyone tho. It's all good, was just lower back that tightened up and I couldn't do my job, let alone move around efficiently. Wouldn't have even went but my work requires a Dr.'s note after a 3rd day. Knowing I could not work I went in on the 2nd day to be looked at and got a work release.

This is nothing like that, went in and got a Toradol shot and excused from work for the rest of the week which at that point was another day. As long as Aflac has been around I'm still surprised as to how unfamiliar facilities are with the service and documents. Going to call my agent here in an hour or 2 and see what he says. He may be able to either talk some sense into them or get Aflac to work around it. I have all the pertinent information already, just don't have the physicians form filled out at their refusal. I really think they think I am trying to file for disability or something.
Think I got it straightened out, have to go home reprint the form and take it back in tonight after work, nurse that I seen said she would fill it out.

This is why Facebook and the like need to go IMO. It is becoming more cumbersome than it is useful. Someone shared one of kids out dancing on graves or something. I didn't watch it as I'm not going to add to their view total. It's like any other behavior. Stop giving these heathens attention and it will stop, the more attention thwy get the more outlandish they become.

Nope, now HR wants to question me on it. Called and asked if it was filled out, "no, I have it but I'll have to get back to you", OK, do you have questions on it or just need to get it filled out? "Yeah, we'll have to sit down and go over a few questions I have". Seriously, if you are an employer and are going to offer something, maybe you should educate yourself on how that product works and understand its processes. If you want to question it when people use it, stop offering it. I work for a living, been here 15+ years, I'm not trying to get free chit.
