    Outside unless I wear the jacket that has the cutout for the strap.

    Depends if you bought it big enough to fit over "heavier" clothes. I really like the pockets/storage mine offers so I always wear mine on the outside.

    Rock Climbing Harness , goes over my pants and I don't have too worry about it being over or under anything....I put jackets on and take them off without it being i the way...
