    No, because I hear it on a regular basis at work. How did it malfunction?

    I'm not the guys at your work.. A release is a tool with moving parts, they are bound to fail eventually.

    couldn't tell you, I'm no release tech. All I can tell you is that it was a second hand Tru Fire 3D Hunter release, kinda junky shape, and the release opened up enough to let my d-loop slip through it.
