I think the inserts are not square in your arrow. Causing the BH to be pointed slightly in one direction or another. Also your arrows might not be perfectly straight. Their is some YouTube videos on making an arrow spinner really cheap. Or you can buy one for like 15 bucks.
How old are the strings? They might have stretched some throwing off cam position, axil to axil might be to long, brace height might be off. It is probably more to do with you though. You are probably frustrated so you are tensing up. Need to relax. Get a repeatable shot sequence ingrained so you don't have to think about it. Shoot at close range say 5 yards don't worry about aiming. Work on one thing at a time like bow hand, anchor point, grip etc. do that till you don't have to think about it and it's all in your subconscious. Then when you want to aim to put all of your focus on where you want the arrow to land. The rest will take care of itself then.
