Name this country song????

Ok I've heard the song a 100 times but can't remember much lyrics

It's a country song, male vocalist playing acoustic guitar, slow sad tempo.

Only verse I can remember he talks about his grandpa getting cancer and wishing he had a chance to see him again before he died. If I remember right the chorus he talks about a old street he remembers, either old Main Street or hwy or something. I really can't remember. Who can help me here? Have I narrowed it down to less than 100 songs lol
